By signing below I agree to the Terms & Conditions. I agree to release and forever discharge and hold harmless to all employees and Voyage Ink Studio from any and all claims, damages or legal actions arising from or connected in any way with my tattoo, or the procedure and conduct used in his/her tattoo. I certify that I am legal guardian of minor, who is willingly submitting to these procedures.

Please include drivers license of both legal guardian and minor and birth certificate of minor. Minor must also fill out Submission Form in its entirety.
